Position Details


* Position: Licensed Vocational Nurse (LVN)


* Employment Type: Full-Time


* Shift: PM Shift


* Location: Moraga Post Acute


* Pay Rate: $38.00 per hour

Responsibilities


* Provide compassionate, high-quality nursing care to residents in accordance with their individualized care plans.


* Administer medications and treatments as ordered by physicians and in accordance with facility policies.


* Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals.


* Perform nursing assessments and document resident care accurately and timely.


* Collaborate with CNAs, RNs, physicians, therapists, and other members of the healthcare team.


* Assist with developing and implementing resident care plans.


* Provide wound care and other treatments as directed.


* Monitor vital signs and other clinical indicators as needed.


* Maintain accurate and complete nursing documentation.


* Communicate effectively with residents, families, and healthcare providers.


* Promote resident safety, dignity, privacy, and independence.


* Follow all facility policies, procedures, and applicable healthcare regulations.


* Respond appropriately to emergencies and changes in resident condition.


* Provide guidance and support to nursing assistants and other care team members as appropriate.


* Perform other nursing-related duties as assigned.

Qualifications


* Current and valid California LVN license.


* Previous skilled nursing or long-term care experience preferred.


* Strong clinical, organizational, and communication skills.


* Knowledge of medication administration, treatments, and nursing procedures.


* Ability to recognize and respond appropriately to changes in resident condition.


* Compassionate and respectful approach to resident care.


* Ability to work effectively as part of a multidisciplinary team.


* Dependable, professional, and able to work independently.


* Strong attention to detail and accurate documentation skills.


* Commitment to providing safe, quality, and person-centered care.
