Program Specialist

Monday-Friday, standard business hours with flexibility for occasional evening or weekend meetings.

Some on-call rotation required.

Requirements: Master's degree in a human services field + 1 year of IDD experience, or Bachelor's degree in a human services field + 2 years of IDD experience (per PA 6400.44) Valid PA clearances: FBI fingerprint, PA State Police background check, and Child Abuse History (Act 151) Strong written and verbal communication skills and comfort with electronic documentation systems (HCSIS, Therap, or similar) Valid driver's license, reliable transportation, and ability to travel regularly between residential sites CPR/First Aid certification required within 90 days of hire Preferred: PA 6400/ODP waiver experience, behavior support planning background, and bilingual Spanish ability (Philadelphia market).

Job Duties: Develop, implement, and monitor Individual Support Plans (ISPs) rooted in person-centered planning principles Facilitate ISP team meetings with individuals, families, Support Coordinators, and providers Ensure compliance with PA Chapter 6400 regulations and maintain audit-ready documentation Coordinate with nursing staff on health monitoring and medical care for individuals with Medically Complex Conditions (MCC) Train and mentor Direct Support Professionals on ISP implementation, data collection, and person-centered practices Conduct site visits, review documentation, and support licensing inspections and quality improvement efforts
