Registered Nurse

HOME HEALTH REGISTERED NURSE (RN) Department: Home Health Position: Registered Nurse (RN) Employment Type: Full-Time / Part-Time / PRN Location: Field-Based / Home Health Visits $5,000 SIGN-ON BONUS! NEW GRADUATES WELCOME TO APPLY! We are seeking a compassionate, dependable, and skilled Registered Nurse (RN) to join our Home Health team.

The Home Health RN provides skilled nursing care to patients in their homes, helping individuals maintain their health, independence, and quality of life while avoiding unnecessary hospitalizations.

The RN will assess patients, develop and implement individualized plans of care, provide skilled nursing services, educate patients and caregivers, and collaborate with physicians and the interdisciplinary care team.

Essential Responsibilities Provide skilled nursing care to patients in their homes according to physician orders and the individualized plan of care.

Perform comprehensive nursing assessments and regularly evaluate patient status.

Develop, implement, and update individualized plans of care.

Monitor patient progress and identify changes in condition requiring intervention.

Communicate significant changes in patient condition to the physician and appropriate members of the care team.

Administer medications and treatments as ordered.

Perform wound care, medication management, injections, catheter care, and other skilled nursing procedures as appropriate to the patient's needs.

Monitor vital signs, symptoms, pain levels, and other clinical indicators.

Provide patient and caregiver education regarding medications, disease processes, treatment plans, safety, nutrition, and self-care.

Coordinate care with physicians, therapists, social workers, aides, and other healthcare professionals.

Participate in interdisciplinary care planning and case conferences.

Complete all required clinical documentation accurately and timely.

Maintain compliance with Medicare, Medicaid, state, federal, agency, and accreditation requirements.

Maintain patient confidentiality and comply with HIPAA regulations.

Follow infection-control, patient-safety, and emergency procedures.

Provide on-call services as assigned, when applicable.

Maintain effective communication with patients, families, caregivers, physicians, and agency staff.

Promote a positive patient experience and represent the organization professionally in the community.

Perform other duties as assigned.

Qualifications Current, unrestricted Registered Nurse (RN) license in the State of Missouri.

Graduate of an accredited nursing program.

New graduates are welcome to apply! Previous home health experience preferred but not required.

Current CPR certification required.

Valid driver's license and reliable transportation.

Current automobile insurance.

Ability to travel to patients' homes throughout the assigned service area.

Ability to work independently while maintaining communication with the Home Health office and clinical team.
