Population Health Assistant (CMA)
Job Title: Population Health Assistant Supervision: Director of Quality Compensation: Non-Exempt Full-Time, based on experience and credentials.
General Summary: Member of the Quality Improvement Department responsible for assisting the Director of Quality with managing all CCHS 3rd party payer programs.
This position also includes working on internal quality improvement initiatives with the QI committee.
The team member will also utilize Athena, 3rd party portals, Azara, Excel, and Word.
Core duties and responsibilities, which include the following and other duties as assigned: Utilize Athena and Azara to run reports and analyze quality data, identify gaps in care, and proactively respond to the findings for CCHS patients.
Work with sites to close patient care gaps by generating reports, sharing gap lists with clinics, and collecting data to close care gaps.
Assist with managing CCHSs 3rd party payer programs such as PEIA Comprehensive Care Program, Highmark, Humana, Aetna, The Health Plan, Wellpoint, Aetna Better Health, and Highmark Health Options.
This includes attending meetings, communicating with point of contacts, and managing data.
Assist with managing Accountable Care Organization requirements (Aledade) by monitoring Aledade overlay activity and communicating with Aledade practice transformation specialists.
Qualifications Skills and Abilities: To perform the Population Health Assistant position successfully, the person must demonstrate core competencies in the following areas: Basic keyboarding and use of Word Navigation of an Electronic Health Record Navigation of a population health tool Proper medical documentation skills Knowledge of Athena, Azara, basic Excel, and Word Ability and willingness to learn, grow, and work independently.
Education and Experience: Licensed or credentialed medical assistant 3-5 years of outpatient, family medicine experience Experience with electronic health record Proficient use of email and online reporting systems Highly organized with the ability to keep accurate notes and records Essential Functions: The employee must be able to perform the essential functions of the position, with or without reasonable accommodation.
