Schedule: 4 Days On, 2 Days Off (Rotating Schedule)

Position Summary

We are seeking a reliable, clinically skilled, and empathetic Licensed Vocational Nurse (LVN) to join our team on the AM shift.

Operating on a stable 4 on, 2 off rotating schedule, you will provide high-quality nursing care, administer medications, and oversee the care team to ensure the highest standards of safety and resident wellness.

Key Responsibilities



* Clinical Nursing: Administer medications, treatments, and ordered therapies according to resident care plans.


* Resident Monitoring: Assess resident conditions, monitor vital signs, and promptly report any changes to the RN or physician.


* Documentation: Maintain accurate, timely electronic medical records (EMR) and shift reports.


* Team Leadership: Supervise and support Certified Nursing Assistants (CNAs) on duty to ensure seamless daily care.


* Communication: Act as a primary point of contact for residents, family members, and attending physicians.

Qualifications



* Active, unencumbered California LVN license.


* Current CPR / BLS certification.


* Strong clinical skills and knowledge of skilled nursing/long-term care regulations.


* Excellent communication, leadership, and problem-solving abilities.


* Experience with PointClickCare or similar EMR systems preferred.
