Electronic Referral Specialist
Electronic Referral SpecialistFull-TimeRemote The Electronic Referral Specialist is responsible for monitoring, retrieving, and managing patient referrals from a variety of electronic referral platforms, including but not limited to CarePort, EPIC, MoreCare, Ensocare, AIDIN, and AIDA.
This role plays a vital part in supporting the admissions workflow by ensuring timely follow-up, accurate documentation, and effective communication with both internal teams and external partners.
Working independently, the specialist must be highly organized, detail-oriented, and responsive in order to prioritize incoming referrals and ensure that no opportunities for care are delayed or missed.
Key Responsibilities: Monitor multiple electronic referral platforms throughout the day for new patient referrals.
Retrieve, review, and process incoming electronic referrals promptly and accurately.
Maintain comprehensive documentation of all referral-related activities in internal systems.
Communicate professionally and effectively with referral sources, internal departments, and external stakeholders to clarify details and coordinate next steps.
Prioritize and manage multiple referrals simultaneously while ensuring timely follow-up.
Escalate urgent, complex, or unclear cases to the Admissions Supervisor as needed.
Maintain up-to-date knowledge of all relevant electronic referral systems and company protocols.
Participate in team meetings, training sessions, and performance evaluations.
Comply with HIPAA regulations and maintain the confidentiality of patient information.
Perform other related duties as assigned.
Qualifications: High school diploma or equivalent required.
Minimum of 1 year of customer service or healthcare-related experience required; experience in home health strongly preferred.
Knowledge of home health service delivery issues is preferred.
Familiarity with Medicare enrollment and home health agency guidelines is a plus.
Ability to type at least 30 words per minute.
Strong analytical thinking and decision-making skills.
Ability to work well under pressure and meet strict deadlines.
Excellent customer service and professional communication skills (written and verbal).
Strong attention to detail with the ability to multitask and manage multiple screens/systems.
Effective time management skills and ability to perform in a fast-paced environment.
High adaptability to change and process updates.
Must be fluent in reading, writing, and speaking English.
Reliable attendance and punctuality are required.
Willingness to work weekends and holidays as needed.
Prior experience in healthcare intake, admissions, or referral coordination is preferred.
Familiarity with platforms such as CarePort, EPIC, MoreCare, Ensocare, AIDIN, or AIDA is highly desirable.
Proficiency in Microsoft Office applications (Outlook, Excel, Word) and adaptability to new software.
Ability to work independently with minimal supervision.
