Position Summary
Liberty Resources, Inc.
is seeking a Board-Certified Physician to join our Federally Qualified Health Center (FQHC) in Syracuse, NY.
This position provides an exciting opportunity to practice patient-centered primary care within an integrated healthcare setting that combines medical and behavioral health services.
As one of only a limited number of organizations nationally designated as both a Certified Community Behavioral Health Clinic (CCBHC) and FQHC, Liberty Resources offers physicians the opportunity to provide whole-person care in a highly collaborative environment focused on improving health outcomes for underserved communities.
The successful candidate will work closely with primary care providers, behavioral health clinicians, nursing staff, care managers, and other healthcare professionals to deliver high-quality, evidence-based care to diverse patient populations.

Responsibilities
Clinical Practice
* Provide direct patient care in an outpatient primary care setting.
* Deliver comprehensive medical services within your area of specialty, including Family Medicine, Internal Medicine, or Med/Peds.
* Diagnose, treat, and manage acute and chronic medical conditions.
* Perform preventive health screenings, annual wellness visits, and health maintenance services.
* Develop and implement individualized treatment plans based on evidence-based practices.
* Collaborate with behavioral health team members to provide integrated care for patients with co-occurring medical and behavioral health conditions.
* Participate in regular case conferences and interdisciplinary team meetings to coordinate care plans for complex patients.
* Coordinate care with specialists, hospitals, and community providers as appropriate.
* Maintain accurate and timely electronic medical records in accordance with organizational standards.
