What You'll Do

As a Physical Therapist, you will provide skilled, resident-centered care focused on improving mobility, function, independence, and quality of life.

Responsibilities include:


* Perform physical therapy evaluations and develop individualized treatment plans.


* Provide skilled interventions to improve mobility, strength, balance, endurance, and functional independence.


* Monitor resident progress and adjust treatment plans as appropriate.


* Collaborate with physicians, nursing staff, therapy professionals, residents, and families.


* Maintain accurate and timely clinical documentation.


* Educate residents and caregivers on therapeutic exercises, mobility strategies, and safety techniques.


* Participate in interdisciplinary care planning and support positive rehabilitation outcomes.


* Provide compassionate, high-quality care in a skilled nursing environment.

Qualifications


* Current California Physical Therapist (PT) license in good standing.


* Strong communication and interpersonal skills.


* Ability to work effectively as part of an interdisciplinary team.


* Strong clinical reasoning and organizational skills.


* Passion for helping residents achieve their rehabilitation goals.
