Position Summary

We are seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis.

In this role, you will evaluate and treat residents with communication, cognitive, and swallowing disorders, helping them achieve their highest level of function, independence, and quality of life.

Essential Duties


* Evaluate residents to determine speech, language, cognitive, and swallowing therapy needs in accordance with physician orders and facility policies


* Develop, implement, and modify individualized treatment plans based on resident goals and progress


* Provide skilled speech-language pathology services to residents


* Assist the nursing department with training of Restorative Aides as appropriate


* Supervise Clinical Fellows (CFY) and students in accordance with state practice regulations


* Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding treatment plans, progress, and discharge recommendations


* Participate in care plan meetings, utilization review meetings, and rehabilitation conferences as needed


* Conduct in-service education and training programs for facility staff


* Complete timely and accurate documentation in accordance with facility policies, regulatory requirements, and payer guidelines


* Maintain a safe treatment environment by following infection control and safety protocols


* Educate residents, families, and caregivers on communication strategies, cognitive support, swallowing precautions, aspiration prevention, and maintenance programs


* Recommend and facilitate appropriate adaptive equipment and interventions to support resident function and safety


* Participate in quality assurance and performance improvement initiatives


* Build positive relationships with residents, families, staff, and professional colleagues
