Position Summary

We are seeking a compassionate and skilled Occupational Therapist (OTR) to provide patient-centered therapy services that improve residents' independence and quality of life.

This role focuses on evaluation, treatment planning, and collaboration with the interdisciplinary care team.
Key Responsibilities


* Evaluate residents and develop individualized treatment plans


* Provide therapy services per physician orders and care goals


* Document evaluations, treatments, and progress notes accurately and timely


* Communicate with physicians, nursing staff, and interdisciplinary team members


* Educate residents, families, and caregivers on therapy programs


* Participate in care planning and discharge planning


* Conduct staff in-services and training


* Maintain compliance with all state, federal, and payer regulations

Qualifications


* Degree in Occupational Therapy (Bachelor's required; Master's or Doctorate preferred)


* Active South Carolina Occupational Therapist (OTR) license (in good standing)


* Experience in skilled nursing or rehab setting preferred


* Familiarity with PointClickCare (PCC) and Casamba preferred


* Strong clinical, documentation, and communication skills
