Position Summary

We are seeking a skilled and compassionate Occupational Therapist (OTR) to provide patient-centered therapy services on a PRN basis.

In this role, you'll help residents improve function, independence, and quality of life while collaborating with an interdisciplinary care team.
Key Responsibilities


* Evaluate residents and develop individualized treatment plans


* Provide therapy services per physician orders and care goals


* Document evaluations, treatments, and progress notes accurately and timely


* Collaborate with physicians, nursing, and therapy team members


* Educate residents, families, and caregivers on therapy programs


* Participate in care planning and discharge planning


* Conduct in-services and staff training as needed


* Ensure compliance with all state, federal, and payer guidelines

Qualifications


* Degree in Occupational Therapy (Bachelor's required; Master's/Doctorate preferred)


* Active South Carolina OT license (OTR) in good standing


* Experience in skilled nursing or rehab setting preferred


* Familiarity with PointClickCare (PCC) and Casamba preferred


* Strong documentation, communication, and clinical skills
