Position Summary:
As a LPN in our NEIGHBORHOOD PACE Program you will be part of a dynamic Interdisciplinary Team (IDT) and work in an innovative model of care with proven positive impacts on helping older adults stay healthy, independent and active in a community health setting.
We have one of the longest standing PACE Programs nationwide (Program of All-Inclusive Care for the Elderly) and are part of an excellent organization that is one of the largest community health centers in the country.
Assess health status and symptoms of ESP participants and record the related health data in the participant’s electronic medical record
* Collects health/symptom data from the participant and/or caregivers
* Navigates the medical record
Participates in analysis and interpretation of recorded data, and in making informed judgments as to the specific elements of nursing care mandated by a particular situation:
* Correlate new data and symptoms with the existing medical records.
* Determines indication for the appropriate response level utilizing consultation from the other team members within established guidelines.
Plan and implement nursing interventions, in consultation with other team members as required by scope of practice or delivery of coordinated care, including all appropriate elements of nurse care and prescribed medical or other therapeutic regimens mandated by an acute or chronic condition, other relevant conditions (such as care giver status, health wishes, etc) scientific principles, recent advancements and current knowledge in the field:
* Review documentation to assure consistency with planned regimens.
* Communicate with the patient’s provider and other relevant members of the IDT
* After communication with a provider, educate the patient or HCP on test results and other findings, proposed changes to care plan, new treatment plans and/or other relevant health care...
