Join Our Team as a Social Services Director!

McKinley Park Care Center is seeking a dynamic, compassionate, and organized Social Services Director to join our team.

This role is ideal for someone who thrives in a fast-paced healthcare environment and is passionate about supporting residents and their families through every stage of care.

What You'll Do


* Develop individualized care plans by working closely with residents and their families


* Support residents in achieving the highest level of independence and well-being


* Provide medically related social services to enhance residents' physical, mental, and psychosocial health


* Assess emotional, psychological, and social needs of residents


* Offer counseling and emotional support to residents and families


* Assist with healthcare decision-making and coping with illness, disability, or loss


* Coordinate discharge planning with interdisciplinary teams and outside agencies


* Connect residents with community resources and support services


* Schedule outpatient appointments and coordinate ancillary services (optometry, podiatry, dental, psychiatric, etc.)


* Maintain accurate and timely documentation


* Manage resident belongings and respond to lost/missing item reports


* Provide staff in-service training as needed


* Build strong relationships with internal teams and external partners

What We're Looking For


* Bachelor's Degree in Social Work or Human Services required


* Minimum of 2 years of supervised social work experience in a healthcare setting


* MSW preferred


* Strong communication, organization, and problem-solving skills


* Ability to handle sensitive situations with professionalism and confidentiality


* A proactive, compassionate approach to resident care
