Job Title: Social Service Designee
Job Summary

The Social Service Designee is responsible for assisting residents/patients and their families with social, emotional, and discharge planning needs.

This role supports overall well-being by coordinating services, advocating for residents, and ensuring compliance with applicable regulations.
Key Responsibilities


* Assess residents' social, emotional, and psychological needs upon admission and throughout their stay


* Develop and implement individualized care plans in collaboration with the interdisciplinary team


* Provide counseling and emotional support to residents and families


* Assist with discharge planning, including coordinating community resources and services


* Maintain accurate and timely documentation in accordance with state and federal regulations


* Advocate for residents' rights and ensure dignity and respect are upheld


* Facilitate communication between residents, families, and healthcare staff


* Coordinate social services programs such as support groups or recreational activities


* Assist with resolving conflicts or concerns involving residents and families

Qualifications


* High school diploma or equivalent required; associate or bachelor's degree in social work, human services, or related field preferred


* Previous experience in healthcare, long-term care, or social services preferred


* Knowledge of basic social service practices and community resources


* Strong interpersonal, communication, and organizational skills


* Ability to handle sensitive information with confidentiality


* Compassionate and patient-centered approach

Work Environment


* Typically works in nursing homes, assisted living facilities, rehabilitation centers, or hospitals


* May involve interaction with elderly, disabled, or chronically ill individuals


* Combination of desk work and direct resident interaction
