Community Health Worker
Salary Range: $23.00-$24.88 per hour
SUMMARY
The Community Health Worker supports Medi-Cal–eligible adults involved in the justice system as part of California’s Medicaid 1115 JI Reentry Initiative.
This role provides outreach, care navigation, and non-clinical case management services to individuals transitioning from incarceration in state prisons, county jails, back into the community.
Working with clients during incarceration, the Community Health Worker helps ensure continuity of Medi-Cal coverage and connection to essential health care services, medications, Enhanced Care Management (ECM), and community-based supportive resources.
The position plays a key role in promoting successful reentry, improving health outcomes, and reducing recidivism through culturally responsive, client-centered support.
ESSENTIAL FUNCTIONS
* Provides release planning services to clients with forensic history, mental illness and/or dual diagnosis.
* Receives and reviews referrals to identify any release planning needs.
* Meets with clients to introduce and explain services; collaborate with client in completing and reviewing resources and the Initial Release Plan (IRP) based on client’s service area; and overall support client to participate in release planning.
* Utilizes various databases and applications to research client historical information, location of client, court date, housing, health status, and level of need.
* Timely documents client encounter in clients’ medical chart.
* Completes internal documentation and reports in a timely manner.
* Assists program development in training and orienting onboarding PRCM staff.
* Completes DHS-CHS department trainings in a timely manner.
* Communicates and coordinates with other community, County and internal stakeholders to support client in accessing resources.
* Assists with other tasks as needed.
JOB QUALIFICATIONS
Ability to follow oral and written instructions.
Ability to exercise sound judgement while working in a stressful environment.
Able to lift up to a 10-pound limit and some bending, stooping, or squatting.
Education/Experience
* Six (6) months of full-time experience working with the public or with community groups performing duties such as interview clients or patients concerning health or social service matters, answering questions, and providing information about health, mental health, and social services to clients or patients.
Certificates/Licenses/Clearances
* Successfully clear the LA County Live Scan process.
* Successfully clear the Health Clearance
