COTA (Certified Occupational Therapist Assistant) - Sherwood Oaks Post Acute

* Provide patient treatment as directed by the Occupational Therapist and per physician's plan of care.


* Document evaluations, daily treatment notes, weekly progress notes, and recertifications per OT Board and payer requirements.


* Communicate patient progress, issues, and plans with supervisor and healthcare team members.


* Assist in training Restorative Aides and instruct patients, families, and nursing staff on maintenance programs for discharge.


* Participate in in-service training programs and discharge planning activities.


* Maintain cleanliness and orderliness of treatment areas; report equipment issues promptly.


* Ensure compliance with state licensure, continuing education, and professional standards.


* Utilize computer programs (e.g., Casamba, Point Click Care) for documentation and data entry.


* Perform job duties requiring frequent standing, walking, lifting (up to 100 lbs occasionally), and other physical demands.


* Work effectively with patients, families, staff, and professional colleagues in a low-to-moderate noise environment.
