Physical Therapist (Per Diem) - Sherwood Oaks Post Acute

* Evaluate patients within 24 hours (or 1 business day) of physician referral; document delays as required.


* Develop treatment plans, obtain physician approval, and treat patients per physician orders.


* Supervise Physical Therapy Assistants, aides, and students per state practice act.


* Train Restorative Aides and instruct families or nursing staff on maintenance programs and discharge preparation.


* Participate in Patient Care Conferences, Utilization Reviews, Rehabilitation Conferences, and in-service training.


* Maintain accurate records: evaluations, daily and weekly notes, progress notes, recertifications, and treatment changes per PT Board/state requirements.


* Recommend, order, and facilitate necessary durable medical equipment for patient mobility and independence.


* Communicate with supervisors and interdisciplinary team regarding patient progress, problems, and plans.


* Ensure proper care, maintenance, and reporting of departmental equipment issues.


* Maintain licensure, continuing education, and compliance with state and facility regulations.
