PTA (Physical Therapy Assistant) - PRN - Sherwood Oaks Post Acute

* Provide direct patient care as directed by the Physical Therapist and physician treatment plans, including documenting daily notes and weekly progress reports.


* Support departmental operations by assisting in cleaning, equipment maintenance, and maintaining a safe and functional treatment environment.


* Collaborate with healthcare team members, participating in care and rehab conferences, communicating patient progress, and assisting with training Restorative Aides.


* Educate patients, families, and nursing staff on maintenance programs and caregiver responsibilities prior to discharge.


* Maintain compliance with state regulations, including continuing education and holding an active, valid PTA license.


* Contribute to staff development through in-service training and knowledge sharing.


* Demonstrate strong interpersonal and communication skills, effectively engaging with patients, families, staff, and colleagues.


* Utilize electronic documentation systems such as Casamba and PointClickCare proficiently.


* Physically capable of demanding tasks, including frequent standing, walking, pushing, pulling, and occasional heavy lifting (up to 100+ lbs).


* Adapt to a moderate-noise work environment, following all policies, procedures, and managerial assignments as needed.
