Lone Tree Post Acute is seeking a dedicated Certified Occupational Therapist Assistant (COTA) to join our in-house therapy team.
As part of our 99-bed skilled nursing facility, you'll work in a supportive, collaborative environment focused on helping residents achieve their highest level of independence and function.

Responsibilities
* Deliver occupational therapy treatments under the direction of the Occupational Therapist
* Collaborate with the interdisciplinary team to support resident goals
* Educate residents, families, and caregivers on therapy programs and safe transitions home
* Assist with discharge planning and continuity of care
* Maintain accurate and timely documentation
Qualifications
* Current California COTA license
* Strong communication and teamwork skills
