Case Manager

Do you have a bachelors degree and at least 1 year experience in geriatrics? Are you an LPN seeking work-life balance? We're hiring experienced LPNs or Social Workers with a bachelors degree and at least one year of geriatric experience (think nursing home or home health).

No experience with ADvantage Case Management? Thats okay, we are willing to train the right person.
