Case Manager IBH

JOB SUMMARY Under the supervision of the Director of Integrated Behavioral Health, the IBH Case Manager will provide a variety of case management services to patients with mental health and substance abuse concerns, by linking patients to clinic and community resources for improved health and wellbeing.

The Case Manager is responsible for evaluating patients needs, contributing to their treatment plan, connecting them with appropriate resources to successfully manage their conditions, documenting interventions, and reporting progress and/or additional needs to the clinical team.

JOB DUTIES AND RESPONSIBILITIES Accept newly assigned cases for which the initial Intake has been completed by a SCHC Behavioral Health Consultant or Drug and Alcohol Counselor.

Completes case management assessment on all patients following an assigned protocolEngages the patient in the development of a service plan tailored to meet the patients unique needs, circumstances, and preferences as determined through assessment.

Educates the patient to make informed choicesWorks actively and collaboratively with patients assigned medical treatment teams to help patients achieve their overall treatment plan goalsProvides ongoing supportive and/or case management functions to patients in accordance with identified problems, needs and strategies.

Arranges, coordinates, and monitors services from qualified resources that are prompt, adequate, and appropriateLinks into a comprehensive network of community resources, and establishes effective working relationships with internal and external resources to facilitate patient needs being metAccompany patients to facilitate access to services or supports, such as applications, appointments, interviews, meetings, etc.

Supports patients ability to communicate his/her needs effectively.Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory standards and requirementsDocument progress, advancement, and completion of goals in a timely and professional manner in the electronic health record according to established standardsParticipates in interdisciplinary team meetings, and other clinic meetings as assignedActively participates in assigned educational activities, which further professional growth and developmentMeets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issuesParticipates in planning and evaluation of case management programOther duties as assigned KNOWLEDGE, SKILLS, AND ABILITIES Knowledge of adult learning theories and group dynamicsKnowledge of various community resources in Shasta County areaAbility to work autonomously and seek out consultation from peers and others when need...
