Licensed Behavioral Health Therapist – LPC/LCSW/LISAC/LMFT (IOP/SUD)

Job Summary iCare Health Services, LLC is seeking an experienced Independently Licensed Behavioral Health Clinician (LPC, LCSW, LISAC, or LMFT) to join our multidisciplinary Integrated Outpatient Behavioral Health team.

The clinician will provide high-quality, person-centered behavioral health and/or substance use disorder services to adults and/or adolescents, based on the clinician's education, training, licensure, competency, and assigned program.

Primary responsibilities may include behavioral health assessments, individual and group psychotherapy, treatment planning, treatment plan reviews, clinical documentation, discharge planning, coordination of care, medical-necessity review, and clinical consultation with members of the multidisciplinary treatment team.

The ideal candidate will have strong experience working with individuals with mental health, substance use, and co-occurring disorders.

Experience with outpatient treatment, Intensive Outpatient Programs (IOP), SUD services, crisis services, or integrated behavioral healthcare is strongly preferred.

Essential Responsibilities Conduct comprehensive behavioral health assessments within professional scope and competency.Provide individual, group, and/or family psychotherapy as appropriate to licensure, training, and assigned program.Develop individualized, measurable, person-centered treatment plans.Complete treatment plan reviews and updates according to clinical need and applicable requirements.Evaluate patient progress and recommend appropriate modifications to treatment.Document medical necessity and clinical rationale for services provided.Complete progress notes and other clinical documentation accurately and within required timelines.Participate in multidisciplinary treatment-team meetings and case consultations.Coordinate care with psychiatric providers, medical providers, nursing, case management, BHTs, peer support staff, and external providers.Identify changes in clinical presentation, risk, functioning, and level-of-care needs and initiate appropriate clinical response.Participate in discharge and transition planning.Provide crisis intervention and safety planning within the clinician's professional scope and organizational protocols.Maintain professional boundaries and patient confidentiality.Maintain compliance with applicable clinical, ethical, organizational, payer, and documentation requirements.Participate in quality-management, utilization-review, chart-review, and performance-improvement activities.Assist with clinical consultation and oversight of behavioral health support staff when appropriately assigned and permitted by credential, competency, and organizational policy.Maintain required continuing education, professional license, credentialing documentation, and training.
