Position Summary
Dublin Post Acute is seeking a compassionate, dependable, and skilled Licensed Practical Nurse (LPN) to join our Day Shift nursing team.
The LPN provides direct nursing care to residents under the supervision of the Registered Nurse (RN) or Director of Nursing while ensuring the highest standards of quality, safety, and resident-centered care.
The ideal candidate is committed to promoting resident dignity, comfort, and well-being in a collaborative post-acute and long-term care environment.
Essential Responsibilities
* Deliver quality nursing care in accordance with physician orders, care plans, and facility policies.
* Administer medications and treatments safely and accurately.
* Monitor residents' physical and emotional condition and promptly report changes to the RN or physician.
* Document assessments, treatments, medications, and resident responses accurately in the electronic medical record.
* Assist in developing and updating individualized care plans.
* Supervise and provide guidance to Certified Nursing Assistants (CNAs) as assigned.
* Respond appropriately to resident needs, emergencies, and changes in condition.
* Communicate effectively with residents, families, physicians, and interdisciplinary team members.
* Maintain compliance with state, federal, and facility regulations.
* Promote infection prevention and control practices.
* Support resident rights while maintaining privacy and confidentiality.
* Participate in shift reports, staff meetings, and continuing education opportunities.
* Foster a positive, respectful, and team-oriented work environment.
Qualifications
* Current, unrestricted Licensed Practical Nurse (LPN) license in the State of Georgia.
* Graduate of an accredited practical nursing program.
* Current CPR/BLS certification (or ability to obtain upon hire).
* Previous experience in skilled nursing, rehabilitation, post-acute care, or long-term care preferred, but new graduates are encouraged to apply.
* Strong clinical assessment and communication skills.
* Ability to prioritize tasks and work efficiently in a fast-paced environment.
* Basic computer and electronic medical record (EMR) proficiency.
Physical Requirements
* Ability to stand and walk for extended periods.
* Frequently lift, transfer, reposition, or assist residents in accordance with safe patient handling guidelines.
* Ability to bend, stoop, reach, and push/pull equipment as necessary.
* Ability to respond quickly in emergency situations.
