Case Manager- CEP

Base Pay: $23.00 - $33.00 / hour JOB SUMMARY: Under the supervision of the Director of Integrated Behavioral Health, the Case Manager will provide a variety of services to patients with housing, mental health and/or substance use concerns who have a history of misdemeanor involvement within the justice system.

The Case Manager is responsible for linking patients to clinic and community resources to improved general health and wellbeing while reducing recidivism.

The position is responsible for evaluating patient's needs, contributing to their treatment plan, connecting them with appropriate resources to successfully manage their conditions, documenting interventions, and reporting progress and/or additional needs to the clinical team.

This role will provide case management services for patients with complex health and social issues including individuals who are unsheltered, have a substance use or mental health diagnosis or are high utilizing Medi-Cal and Medi-Medi beneficiaries.

The Case Manager will support accessing and linking patients to resources in the community.

The Case Manager is responsible for enrolling appropriate participants, assessment, making appropriate referrals, appointment setting, and monitoring/reporting patients' progress and needs, patient empowerment and utilization review and fieldwork.

The Case Manager will work closely with the patient's integrated team, community non-profits, hospital, courts, probation and health clinics to coordinate access to needs.

The Case Manager will also be responsible for following SCHC protocol and practices around billing, documenting case notes, and entering data regarding patient's progress in required Case Management system.

JOB DUTIES AND RESPONSIBILITIES • Performs a variety of services related to the case management of patients • Reviews daily referrals received from justice partners, the court systems and other outside and in-house referrals; meet with clients in order to obtain program consent, administer required assessments, and review client's care plan, medical follow-up and psychosocial needs.
