Key Responsibilities

Treatment & Resident Care


* Deliver high-quality physical therapy interventions under the direction and supervision of a licensed Physical Therapist.


* Provide individualized treatment focused on mobility, strength, balance, functional independence, and quality of life.


* Monitor resident responses to treatment and communicate progress or concerns to the supervising PT.

Documentation & Progress Monitoring


* Maintain accurate and timely clinical documentation.


* Track resident progress and support achievement of established treatment goals.


* Follow facility policies, clinical standards, and applicable regulatory requirements.

Interdisciplinary Collaboration


* Collaborate with Nursing, Activities, and the broader Therapy team to support comprehensive resident care.


* Participate in care planning and communicate effectively with interdisciplinary team members.

Resident & Caregiver Education


* Educate residents and caregivers on safe mobility techniques, exercises, and strategies to promote independence.


* Support caregivers and staff with carryover of appropriate therapeutic and restorative techniques.

Qualifications


* Current, valid California Physical Therapist Assistant (PTA) License required.


* Strong communication and interpersonal skills.


* Team-oriented mindset with a passion for working with residents in a skilled nursing and post-acute setting.
