Join our team and make a difference in your community! Job Description The Occupational Therapist assesses and treats patients in the home in order to assists them in the development and execution of activities of daily living.

Evaluates patients functional status as well as the home environment to identify hazards or barriers to achieve more independent livingPerforms appropriate diagnostic musculoskeletal, neurological and functional skills tests to develop a treatment plan that establishes goals for improved functionCommunicates plan of care to referring physician and collaborates with other clinicians to ensure patient needs are metEducates patients on the use of various types of adaptive equipment to improve functionProvides instruction on proper posture/ body mechanics to promote prevention of injury and maximize therapeutic outcomesEducates patients concerning how to maximize quality of life within the limits of their physical capabilitiesProvides guidance and education to family members and caregivers to ensure patients continue to sustain improvement with their physical, mental or cognitive disordersDocuments all changes in patients condition and updates care plans and outcomes accordinglySupervises the care provided by Certified Occupational Therapy Assistants to ensure proper care is deliveredInstructs, supervises, and evaluates home health aide care every two (2) weeks when therapy is the only skilled serviceParticipates in discharge planning for the patient
