Concord Post Acute is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our dynamic rehabilitation team on a PRN/per-diem basis.

Position Details


* Position: Physical Therapist Assistant (PTA)


* Employment Type: PRN / Per-Diem


* Location: Concord, CA


* Compensation: Starting at $50-$53/hour (DOE)


* Department: In-House Rehabilitation Team

Key Responsibilities


* Provide skilled physical therapy treatments under the direction of the supervising Physical Therapist


* Accurately document daily treatments, progress notes, and patient outcomes


* Collaborate with nursing staff, therapists, and interdisciplinary team members regarding resident care plans


* Participate in patient care conferences and ongoing treatment planning


* Assist in training and supporting Restorative Aides to promote continuity of care


* Educate patients, families, and caregivers on maintenance programs and safe discharge planning


* Maintain professional, compassionate communication with residents, families, and staff

Qualifications


* Active California Physical Therapist Assistant (PTA) license in good standing
