Telehealth Follow-Up Nurse (LPN) - Remote!

Job Description: As a Follow Up Care Coordinator at MetaPhy, you will play a crucial role in delivering patient education, support, and care through our telehealth platform.

You will be responsible for a caseload of 250-260 patients monthly with a minimum completion of 10 successful patient calls per day.

This is a performance-based role: meeting the weekly quota is a core expectation, regardless of how many outreach calls or follow-ups are required to achieve it.

Success in this role depends on persistence, effective communication, and a proactive approach to patient engagement.
