What Youll Do Support Person-Centered Planning & Care Establish objectives for the Person-Centered Individualized Support Plan (PCISP) goalsAssist with the development and approval of Behavior Support Plans (BSPs) Prepare high-risk plans in partnership with the Wellness Coordinator (RN) Write individualized protocols and complete monthly and quarterly reviews Ensure Quality, Safety & Compliance Report and present information for Human Rights Committee meetings Ensure utility bills, leases, and documentation are handled correctly Review end-of-month documentation for accuracy Attend appointments and administer medications as needed Lead & Support DSP Teams Provide guidance and support to Direct Support Professionals (DSPs) Conduct performance evaluations Manage attendance, approve timecards, and assign attendance points Assist DSPs with organizing shift responsibilities Provide Coverage & Support Fill open shifts and provide shift coverage when needed Provide on-call support based on service site needs This role is well-suited for someone who enjoys leadership, teamwork, and working in a mission-driven environment.
