Victorian Post Acute is seeking a compassionate and motivated Physical Therapist to join our growing in-house rehabilitation team.

Key Responsibilities


* Evaluate patients and develop individualized treatment plans.


* Provide skilled physical therapy services in accordance with physician orders and established care plans.


* Supervise Physical Therapist Assistants and ensure compliance with state practice regulations.


* Collaborate with interdisciplinary team members to coordinate and optimize patient care.


* Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.


* Maintain timely and accurate documentation, including evaluations, daily treatment notes, progress reports, and discharge summaries.


* Educate patients, families, and caregivers on treatment goals, home exercise programs, and safe mobility techniques.


* Assist with discharge planning and recommendations for continued care.


* Recommend and facilitate acquisition of durable medical equipment as appropriate.


* Monitor and report equipment maintenance needs to ensure a safe treatment environment.

Qualifications


* Current Physical Therapist license in good standing.


* Strong communication and interpersonal skills.


* Commitment to providing exceptional patient-centered care.
