What You'll Do:


* Evaluate residents to determine therapy needs and create individualized treatment plans


* Provide direct physical therapy treatments in accordance with physician orders and resident goals


* Document evaluations, progress notes, and treatment records accurately and timely


* Collaborate with an interdisciplinary team to ensure comprehensive resident care


* Supervise and support Physical Therapy Assistants (PTAs)


* Participate in discharge planning and caregiver education to ensure continuity of care


* Maintain compliance with state, federal, and facility regulations

What We're Looking For:


* Licensed Physical Therapist (PT) in the State of California


* Strong clinical skills and dedication to resident-centered care


* Ability to work independently as well as part of a team


* Excellent communication and organizational skills


* Skilled nursing or post-acute rehab experience preferred, but not required
