Medical Records Assistant (Part-Time) - Hayward Gardens Post Acute

Hayward Gardens Post Acute is looking for a detail-oriented and organized Medical Records Assistant to join our team on a part-time basis.

This role is vital to ensuring our health information systems are accurate, secure, and compliant with all regulatory standards.

Rate of Pay: $19 - $20 per hour

The primary purpose of your position is to maintain resident medical records and health information systems.

You will ensure all documentation is handled in accordance with federal and state guidelines, as well as our facility's established privacy policies and procedures.

Key Responsibilities

Health Information Management


* Chart Maintenance: Collect, assemble, check, and file resident charts.

Ensure all information—including nursing notes, assessments, lab reports, and X-ray results—is filed accurately into resident records.


* Quality Control: Identify incomplete records and return them to the appropriate departments for correction.

Assist in developing procedures to ensure records are properly signed, indexed, and coded before filing.


* Record Retrieval: Retrieve manual or electronic records for authorized personnel (physicians, nurses, and government agencies) and ensure a proper sign-out/sign-in process is followed.

Administrative Support


* Coordination: Assist the MDS Coordinator with scheduling assessments and support the Medical Records Consultant as required.


* Reporting: Abstract authorized information for insurance companies, Medicare, Medicaid, and the VA in strict accordance with Privacy Rules.


* Data Entry: Batch resident information into the computer system and retrieve demographic data as instructed.


* Registry Management: Maintain admission and discharge registries and other departmental files as directed.

Privacy & Security


* Confidentiality: Maintain strict protection of resident health information.

Safeguard your user ID and passwords and report any unauthorized attempts to access the facility's information systems.


* Compliance: Ensure no records leave the medical records room unless specifically authorized by facility policy.

Essential Requirements


* Communication: Maintain a professional rapport with inter-departmental personnel and handle telephone inquiries regarding medical records functions.


* Organization: Must be able to follow work schedules and instructions as outlined by the department supervisor.


* Documentation: Record and file all incidents/accidents according to established procedures and maintain minutes for departmental meetings.
