Full Job Description

Canyon Springs Post-Acute is a 199-bed skilled nursing and short-term rehabilitation facility.

We are seeking a full-time RN Supervisor to oversee daily clinical floor operations, manage staff, and ensure excellent resident care.

Responsibilities:


* Supervise daily floor nursing staff and assign clinical duties.


* Lead morning stand-up meetings to coordinate team communication.


* Update residents and families on Care Plans, PT, OT, and treatment paths.


* Answer clinical, care, and illness progression questions to reassure residents.


* Assess and adjust patient care statuses based on changing clinical needs.


* Conduct utilization reviews to provide timely, cost-effective care.


* Complete accurate, timely charting and paperwork using PointClickCare (PCC).


* Manage admissions, discharge planning, and external facility referrals.


* Coordinate benefits with financial services, Medicare, Medi-Cal, and third-party payers.

Requirements:


* Active California Registered Nurse (RN) license.


* Minimum 2 years of clinical nursing experience (supervisory experience preferred).


* Knowledge of Medicare, Medi-Cal, and Medicaid programs.


* Proficiency with PointClickCare (PCC) or electronic medical records software.


* Excellent communication, problem-solving, and time management skills.


* Good standing with the California Department of Public Health.


* Ability to stand/walk frequently and lift up to 50 lbs.
