Position Summary

As a Physical Therapist, you will evaluate, plan, and implement individualized treatment programs for residents in a skilled nursing and post-acute setting.

You'll play a key role in helping patients recover from injury, surgery, or illness while maximizing their mobility and independence.

Key Responsibilities


* Conduct comprehensive patient evaluations and develop customized care plans


* Provide therapeutic exercises, manual therapy, and mobility training


* Monitor patient progress and adjust treatment plans as needed


* Collaborate with physicians, nursing staff, and therapy team members


* Educate patients and families on treatment plans and home exercises


* Maintain accurate and timely documentation in compliance with regulations

Qualifications


* Active Physical Therapist (PT) license in California


* Graduate of an accredited Physical Therapy program


* Strong clinical assessment and communication skills


* Experience in skilled nursing or post-acute care preferred, but not required


* Commitment to compassionate, high-quality patient care
