The Opportunity


* Location: Concord, CA (Contra Costa County)


* Compensation: Starting at $60.00/hour (DOE)


* Availability: Full-Time, Part-Time, and PRN

Your Impact & Responsibilities


* Clinical Leadership: Perform evaluations and develop physician-approved treatment plans tailored to each resident's unique recovery journey.


* Team Supervision: Provide expert supervision of PTAs in accordance with the California State Practice Act.


* Interdisciplinary Teamwork: Partner with medical and nursing staff to communicate goals, progress, and discharge strategies.


* Resident Advocacy: Lead discharge planning and recommend DME to ensure a safe and successful transition home.


* Education: Empower nursing staff, patients, and families with vital safety and maintenance education.

Qualifications


* Current Physical Therapist (PT) license in the State of California.
