Pay Range: $57-$67 per hour

Position Summary

Sunnyvale Gardens Post Acute is seeking a PRN Physical Therapist to join our skilled nursing and post-acute care team.

The ideal candidate is compassionate, clinically strong, and committed to helping patients restore function, mobility, and quality of life.

Responsibilities


* Evaluate patients and develop individualized treatment plans


* Provide therapeutic exercise, gait training, balance training, and functional mobility interventions


* Document evaluations, treatments, goals, and progress in accordance with regulatory standards


* Collaborate with an interdisciplinary care team to optimize patient outcomes


* Educate patients and families on treatment plans, safety, and home exercise programs


* Maintain compliance with state, federal, and facility policies

Qualifications


* Current California Physical Therapist license in good standing


* Graduate of an accredited Physical Therapy program


* Experience in skilled nursing or post-acute care preferred


* Strong communication, clinical, and documentation skills


* Ability to work independently and as part of a team

Schedule & Compensation


* PRN / Per Diem (flexible scheduling)


* $57-$67 per hour, based on experience
