The Opportunity:



* Position: Occupational Therapist (OTR)


* Location: White Blossom Care Center — San Jose, CA


* Compensation: Up to $65.00 per hour (DOE)


* Availability: Full-Time, Part-Time, and PRN


* Program: Stable, In-House Therapy (Not a contract agency)

Your Impact & Responsibilities:



* Evaluations & Treatment: Develop effective, evidence-based treatment plans and provide skilled interventions as prescribed by referring physicians.


* Clinical Supervision: Provide leadership and supervision to Occupational Therapy Assistants (COTAs) in accordance with California State Practice Acts.


* Interdisciplinary Collaboration: Participate in Patient Care Conferences, Utilization Review, and Rehabilitation meetings to ensure holistic resident care.


* Expert Documentation: Maintain precise records, including evaluations, daily notes, and recertification documentation per OT Board requirements.


* Independence & Equipment: Facilitate the ordering and training of durable medical equipment (DME) such as braces, walkers, and ADL aids to promote patient safety and independence.


* Education: Instruct families and nursing staff in maintenance programs and caregiver training to ensure successful discharge planning.
