Your Impact & Responsibilities:



* Comprehensive Evaluations: Perform high-quality physical therapy evaluations, developing personalized treatment plans for post-acute and long-term care residents.


* Skilled Interventions: Implement evidence-based treatments to improve resident mobility, balance, strength, and overall functional independence.


* IDT Collaboration: Partner closely with nursing, therapy, and medical providers to ensure a holistic approach to resident recovery.


* Expert Documentation: Maintain precise and timely documentation in accordance with California state and federal regulatory requirements.


* Patient Advocacy: Educate residents and their families on individualized therapy programs, safety protocols, and long-term functional goals.
