Key Responsibilities


* Evaluate patients and develop individualized treatment plans


* Deliver skilled physical therapy services to improve mobility, strength, and function


* Collaborate with nursing and rehab teams to ensure optimal outcomes


* Maintain accurate and timely documentation per SNF and regulatory standards

Qualifications


* Active California Physical Therapist (PT) license


* Strong clinical decision-making and communication skills


* Passion for working with geriatric and post-acute populations


* SNF experience preferred, but new grads are welcome to apply
