Position Details


* Title: MDS Coordinator (LVN)


* Schedule: Part-Time


* Pay Range: $35.00 - $42.00 per hour, depending on experience

What You'll Do


* Coordinate and complete accurate, timely MDS assessments in compliance with state and federal regulations


* Ensure care plans reflect resident needs, clinical status, and regulatory requirements


* Collaborate with interdisciplinary team members to support quality outcomes and reimbursement accuracy


* Monitor documentation for completeness and compliance


* Support survey readiness and quality improvement initiatives


* Serve as a clinical resource related to MDS processes and best practices

What We're Looking For


* Current, active LVN license in good standing


* Prior experience with MDS coordination in a skilled nursing or long-term care setting


* Strong knowledge of Medicare, Medicaid, and RAI processes


* Excellent organizational skills and attention to detail


* Ability to work independently while collaborating with a multidisciplinary team


* Strong communication skills and a positive, solutions-focused mindset
