Key Responsibilities:

🔹 Assess patients and develop individualized treatment plans to improve movement and function
🔹 Provide hands-on therapy and exercises to enhance strength, mobility, and balance
🔹 Educate patients and caregivers on rehabilitation techniques and injury prevention
🔹 Document patient progress and collaborate with interdisciplinary healthcare teams
🔹 Ensure compliance with facility policies and state/federal regulations

Qualifications:

✔ Active Physical Therapist (PT) license in CA
✔ Experience in skilled nursing, rehabilitation, or long-term care preferred
✔ Strong communication, patient-care, and teamwork skills
✔ Passion for helping patients achieve their highest level of independence
