As an MDS Coordinator, you will play a pivotal role in ensuring compliance with federal and state regulations regarding patient assessments, care planning, and documentation processes.

Your expertise will be critical in overseeing the completion of the Minimum Data Set (MDS) assessments and ensuring that our facility meets the highest standards of care.

You will collaborate closely with interdisciplinary teams to develop individualized care plans, conduct comprehensive assessments, and monitor outcomes to enhance patient quality of life.

Responsibilities


* Oversee the completion of MDS assessments and ensure timely submission to state and federal agencies.


* Coordinate interdisciplinary team meetings to develop and review individualized care plans for residents.


* Conduct staff training on MDS procedures and regulatory compliance to ensure all standards are met.


* Review and analyze data from MDS assessments to identify trends and improvement opportunities in care delivery.


* Maintain accurate records and documentation to support compliance with regulatory standards.


* Collaborate with healthcare professionals to ensure that assessments align with residents' needs and preferences.


* Monitor outcomes of care plans and make necessary adjustments to improve quality of care and resident satisfaction.

Requirements
Bachelor's degree in Nursing, Healthcare Administration, or a related field is preferred.



* Current licensure as a Registered Nurse (RN).


* Minimum of two years of experience in long-term care or skilled nursing facility is preferred.


* Strong knowledge of MDS 3.0 guidelines, federal and state regulatory requirements.


* Excellent organizational, analytical, and problem-solving skills.
