CNA - Hospice
General Purpose
The CNA - Hospice role support patients and families with dignity, empathy, and professionalism, ensuring comfort and quality of life during a deeply meaningful stage of care.
Essential Duties
* Provide personal care, hygiene support, exercise assistance, and patient ambulation in accordance with state and federal regulations
* Utilize proper safety techniques and body mechanics when delivering care, transferring patients, and performing daily tasks
* Take and record vital signs and other clinical metrics as directed
* Observe, document, and promptly report changes in the patient's physical or mental condition to the RN Case Manager
* Participate in scheduled weekend rotation for patient visits
* Prepare light meals, perform laundry, and complete essential household tasks that support the patient's care plan
* Maintain required annual continuing education (12 hours) to ensure compliance and professional growth
Education and/or Experience
* Successful completion of a state‑approved Certified Nurse Aide program with active CNA certification
* In good standing with all applicable state regulatory requirements
* Strong communication and interpersonal skills, with the ability to provide compassionate support to patients and families
* Ability to work collaboratively within an interdisciplinary team as well as independently with minimal supervision
* Proficiency with basic computer and technology tools used for documentation and communication
Physical Demands
* Ability to safely lift, move, and transfer patients using proper body mechanics; may require lifting or supporting up to 50 pounds
* Frequent standing, walking, bending, kneeling, and reaching during the course of patient care
* Ability to assist with repositioning, ambulation, and mobility support in a variety of home environments
* Sufficient stamina to perform continuous physical activity throughout the shift, including during scheduled weekend rotations
* Ability to operate basic medical equipment and household items as part of patient care
* Visual, auditory, and tactile ability to monitor patient condition, observe changes, and perform assigned tasks safely
* Ability to travel between patient homes as required
