Key Responsibilities


* Assist the Physical Therapist in developing and implementing individualized treatment plans


* Provide hands-on therapy to help residents improve mobility, strength, balance, and functional independence


* Monitor resident progress and accurately document treatments and responses


* Communicate resident updates and progress with the rehabilitation and interdisciplinary care teams


* Collaborate with nursing and clinical staff to ensure comprehensive, coordinated care


* Maintain compliance with facility policies, procedures, and all state and federal regulations

Qualifications


* Current Physical Therapist Assistant (PTA) license in the state of California


* Previous experience in a skilled nursing or rehabilitation setting preferred


* Strong communication, documentation, and teamwork skills


* Compassionate, motivated, and committed to resident-centered care


* Ability to work effectively in a fast-paced healthcare environment
