CNA or HHA!

The Resident Care Coordinator plays a key role in supporting the residents’ well-being by managing healthcare coordination, advocating for psychosocial needs, while supporting health and wellness initiatives. 

 Skills Needed:


* Clinical and Human Services knowledge:  Understanding psychosocial assessments and interventions. 


* Interpersonal Communication:  Empathetic engagement with residents and families.

Clear, effective communication with interdisciplinary teams.

Conflict resolution and mediation skills. 


* Supportive Presence:  Creating a comforting and engaging atmosphere for our residents.


* Collaboration:  Works with and promotes collaboration among departments, professionals, consultants, and organizations including government regulatory agencies.


* Teamwork:  The ability to work towards a common goal of excellent care for our residents.

Requirements:


* Bachelor’s degree in social work or in a human services field or Social Service Designee certification required.


* Strong passion for geriatric advocacy and commitment to senior care excellence.


* Previous social services experience preferred.



* Demonstrates C.A.R.E.
