What You'll Do:


* Support residents and families with care planning and social needs


* Assist with admissions, discharges, and transitions


* Be a friendly face and a listening ear for our residents


* Help coordinate resources and services to improve quality of life

What We're Looking For:

* Strong communication and problem-solving skills


* Previous experience in social services or healthcare is a plus (but not required!)
