SCOPE OF ROLE

Reporting to the Manager, Ryan White Program, the Team Lead, Ryan White Program will provide a range of client-centered services that link clients with health care, psychosocial and other services, as well as provide oversight and clinical direction for social work and case management professionals.

The Team Lead works closely with the Program Manager to ensure the highest quality of treatment and counseling services are provided to clients to address mental illness, eliminate barriers to treatment and increase adherence to medical care for eligible individuals living with HIV/AIDS.

The Team Lead will work with direct reports, as well as carry a small caseload of Ryan White clients.

HOW YOU'LL SUCCEED:


* Assist the Program Manager in oversight of daily objectives of the Ryan White team members.


* Assist the Program Manager in monitoring the individual performance of team members through coaching, development, and meeting objectives.


* Conduct peer reviews, utilization review and level of care reviews.


* Manage patient census and tracking.


* Oversee the admission, transition and discharge planning for the team.


* Monitor and enforce clients' rights and confidentiality and ensure high ethical standards.


* Participate in all Ryan White Program training and educational requirements


* For a small caseload of clients, actively fulfill Medical Case Manager needs by working at various Signature Health locations as scheduled by your Supervisor.


* Provide an initial assessment of client service needs.


* Develop a comprehensive, individualized care plan and coordinate services required to implement the plan.


* Continuously monitor client progress to assess the efficacy of their treatment plan and conduct periodic re-evaluation and adaptation of the plan at least every six (6) months, as necessary.
