QA/MDS Nurse (LVN) - Lakeport Post Acute

Location: Lakeport, CA
Pay: Starting at $42 per hour, DOE
Schedule: PRN

Responsibilities


* Coordinate and oversee MDS (Minimum Data Set) assessments and submissions


* Ensure compliance with state and federal regulatory requirements


* Collaborate with the interdisciplinary team to support accurate care planning


* Monitor and evaluate quality indicators and assist with QAPI initiatives


* Provide education and guidance to staff on documentation and quality assurance processes


* Assist in audits, surveys, and care plan meetings

Qualifications


* Active California LVN license required


* Prior MDS/QA experience in a skilled nursing facility strongly preferred


* Strong knowledge of state and federal regulations related to MDS and QA


* Excellent assessment, documentation, and organizational skills


* Team-oriented, with strong communication and leadership abilities
