Medical Hill Healthcare is seeking a compassionate and experienced Social Services Director to join our team.

In this full-time, salaried position, you will lead our social services programs, advocating for residents and ensuring their emotional and social needs are met.

Role & Responsibilities:


* Design and manage programs that enhance residents' independence and dignity.


* Provide emotional support and counseling to residents and families.


* Manage and coordinate discharge planning with the care team and external agencies.


* Arrange all on-site ancillary services and outpatient appointments.


* Supervise resident personal shopping and property inventory.


* Serve as the point person for managing incidents involving lost or stolen items.


* Communicate effectively and empathetically with residents, families, and staff.

Qualifications:


* Bachelor's degree in Social Work or Human Services.


* 2+ years of supervised social work experience in a healthcare setting.

MSW is a plus.


* Ability to handle complex, confidential situations with objectivity.


* Excellent organizational, planning, and communication skills.


* Proactive approach to problem-solving.
