The Opportunity


* Seeking Family Care or Primary Care physician assistants and nurse practitioners.


* Current national certification, DEA, and BLS are required.


* At least one (1) year of primary care experience is required.


* Fingerprinting via DOJ will be required.


* Current CA state license is required.

San Bernardino Health Center - San Bernardino, California

Adelanto Community Health Center - Adelanto, California

Ontario Health Center - Ontario, California


* A Federally Qualified Health Center.


* Referral resources include WIC, Dental, Psych, Nutritionist, and a MediCal case worker.


* Facility hours for this location are Mon-Fri 8am-5pm.


* Good mix of patients including peds.


* 15-18 patients per day per provider.


* Point of Care testing available at site; minimal labs.
